Step 1: Select Coverage type. If you intend to only cover yourself under health insurance please select "Employee Only". If you plan to also cover a Spouse please select "Employee + Spouse". If you plan to cover Depedents, but not a Spouse, please select "Employee + Child(ren). If you plan to cover your whole family, including Spouse and Dependents please select "Family".
Step 2: Indicate if your spouse is offered coverage through their employer. Note - If they do and you wish to cover them under USS insurance, you will pay a surcharge of $600 , annually via payroll contributions for health insurance.
Step 3: Please select if you are 55 years or older. If you are, you are entitled to contribute an additional $1,000 into your Health Savings Account above the IRS limit.
Step 4: Please select the amount of money you would like to model contributing into a Health Savings Account. These are pre-tax funds can be used to pay for healthcare expenses. The amount cannot exceed $3,550 for a single or $7,100 for family coverage.
Step 5: Select via the drop down if you are expecting any of the listed procedures in 2021.
Step 6: Select via the drop down if you expect or know you will be taking any regular medications
Step 7: Under "Utilization of Covered Services" you have the option to select the button that best fits the medical care needs of you or members of your family. A Low utilizer would be someone who only goes to the doctors a few times a year. A medium utilizer would be someone who might have a few sick visits, but also sees a specialist like a chiropractor/dermatologist/orthopedic. A medium utilizer would also be someone who might take a reoccuring medication. A high utilizer would experience similar medical care as the Low or High utilizer, but would also expect a surgery.
Step 8: You have the option to type in the visit count for the services listed to create a custom option that best describes the medical care typically needed for you and your family.
